Athletic Forms

All athletes in grades 7-12 must have a current physical on file at LCC in order to participate in athletics. Athletes must have a NEW physical every year.  Below are links to all the forms that must be completed, signed by a parent/guardian and taken to your physician’s office when your son/daughter receives their physical.
This is the letter attached to the packet of information that the athletes can pick up in the general office at LCC.  The letter details why and how the forms are to be completed.
This form must be completed and signed by the athlete and parent/guardian.  This form is required because the Athletic Trainer is employed by St. Rita’s.

 

 

 

This form consists of 6 pages.  Page 1 & 2 must be completed and signed by the athlete and parent/guardian, Page 3 & 4 is to be completed, signed and DATED by the Physician, and pages 5 & 6 must be signed by both the athlete and the parent/guardian.

Interscholastic Concussion Form
According to the recently passed concussion bill, this form must be read and signed by the athlete as well as their parent/guardian for each sport they participate in.

Please return all completed forms to the General Office at LCC, Frank Kill A.D, or Stephanie Watkins.
